As an applicant for a new license or a change to an
existing license, you are required to complete certain tasks and provide certain documentation
before the license can be granted. You will be assigned an investigator who will manage your
application through completion and issuance of your license. At your first meeting with your
assigned investigator, you will be provided a checklist, detailing the requirements of the license
for which you've applied.
From the list below you may obtain an advance copy of that checklist in preparation for your
meeting with the investigator. You may obtain a copy of the appropriate application form. Obtaining and reviewing
these in advance will help speed the process to completion, providing a license for you at the
earliest possible date.
