Who needs a permit?
Anyone who serves food to the public is required to have a permit.

Below you can find documents related to a variety of food establishments. Many of the documents require Adobe Acrobat.

How do I get a permit?
Fill out the appropriate application based on your type of operation. Make sure to fill this out completely and submit to the Health Department for review. Choose from one of the following permit applications.

You must return your application to the Health Department at least 30 days prior to your planned opening date.

Please note: Filling out an application does not guarantee you permission to operate. You must contact the Health Department and speak with a food inspector in order to complete the application process.

Once I obtain a permit, is there a need for me to ever go through the process again?

Yes. Permits are issued annually. Every year the Health Department will invoice your restaurant for a permit renewal when it is nearing the permit expiration date. Return this invoice with your payment promptly to ensure your establishment remains permitted.
